W-9 Form for US Citizens

Filling out a W9 form is straightforward.

Line 1 – Name

  • Enter your full name as it appears on your tax return.

Line 2 – Business Name

  • Enter your business name if you have one. If not, leave it blank.

Line 3 – Federal Tax Classification

  • Check the box that describes your tax status:

    • Individual/sole proprietor/single-member LLC

    • C corporation

    • S corporation

    • Partnership

    • Trust/estate

    • LLC: If your LLC is taxed as a partnership, write “P.” If it’s taxed as a corporation, write “C” or “S.”

Line 4 – Exemptions

  • Most individuals leave this blank. If your entity is exempt from backup withholding or FACTA reporting, enter the appropriate code.

Lines 5 & 6 – Address

  • Enter your address where your employer will mail tax information, including city, state, and ZIP code.

Line 7 – Account Number(s)

  • Optional. You can provide account numbers if needed, but most can leave it blank.

Part I – Taxpayer Identification Number (TIN)

  • Enter your Social Security Number (SSN) if you’re an individual or single-member LLC.

  • Use your Employer Identification Number (EIN) if you’re a multi-member LLC or corporation.

  • Sole proprietors can use either SSN or EIN, but SSN is preferred.

  • Resident aliens without an SSN should use their IRS Individual Taxpayer Identification Number (ITIN).

Part II – Certification

  • Sign and date the form to certify the information is accurate.

Part I – Identification of Beneficial Owner

Line 1 – Name of Beneficial Owner

  • Enter your full name as it appears on your official identification documents.

Line 2 – Country of Citizenship

  • Provide the country where you are a citizen.

Line 3 – Permanent Residence Address

  • Enter your permanent residential address. This should be your actual residence, not a P.O. box.

Line 4 – Mailing Address (if different from above)

  • If your mailing address is different from your permanent address, enter it here.

Line 5 – U.S. Taxpayer Identification Number (TIN) (optional)

  • If you have a U.S. TIN (such as an SSN or ITIN), you can enter it here. If not, leave this line blank.

Line 6 – Foreign Tax Identification Number (FTIN)

  • Enter the tax identification number issued by your country of residence if applicable.

Line 7 – Reference Number(s)

  • This line is optional. Enter any reference number that your withholding agent requires you to provide.

Line 8 – Date of Birth

  • Enter your date of birth in MM-DD-YYYY format.

Part II – Claim of Tax Treaty Benefits (if applicable)

Line 9 – Country of Residence for Tax Purposes

  • Enter the name of the country where you are considered a tax resident under the laws of that country.

Line 10 – Special Rates and Conditions (if applicable)

  • If you are claiming special treaty benefits (such as a reduced rate of withholding tax), enter the article and paragraph number of the tax treaty that applies, the rate of withholding tax, and provide a brief explanation of why you qualify for these benefits.

Part III – Certification

  • Sign and date the form to certify that you are the beneficial owner of the income, the information provided is accurate, and you are eligible for any benefits claimed under an income tax treaty with the United States.
